Hereditary angioedema (HAE) is a rare but potentially life-threatening autosomal dominant genetic disorder caused by deficiency or dysfunction of the C1 esterase inhibitor (C1-INH) protein. HAE affects an estimated 1 in 50,000 individuals globally, with a pooled global prevalence of 1.22 cases per 100,000 people based on a systematic review and meta-analysis of 24 studies published between 2000 and 2024, including 11,245 HAE cases. The disease is characterized by recurrent, unpredictable attacks of severe swelling affecting the abdomen, extremities, face, and airways. Without effective therapeutic intervention, laryngeal edema carries a mortality risk of 20–25%. In the United States alone, HAE episodes result in an estimated 15,000 to 30,000 annual emergency department admissions. This growing recognition of the disease burden, combined with improved diagnostic capabilities and rising healthcare awareness, is significantly driving demand for HAE therapeutics globally.
The HAE therapeutics landscape has witnessed an unprecedented wave of regulatory approvals in 2024–2025. The U.S. Food and Drug Administration (FDA) approved EKTERLY (siderostat) in July 2025, the first and only oral plasma kallikrein inhibitor for on-demand treatment of acute HAE attacks in patients aged 12 years and older, offering a major shift from injection-based options. In August 2025, the FDA approved DAWNZERA (dominators), the first RNA-targeted medicine for HAE prophylaxis. Additionally, ANDEMBRY (garadacimab-gxii), the first once-monthly subcutaneous Factor XIIa inhibitor for HAE prophylaxis, was approved by the FDA in June 2025. The European Commission granted marketing authorization to EKTERLY in September 2025 (with Germany launch in Q4 2025) and to DAWNZERA in January 2026. These landmark approvals are substantially expanding the addressable patient population and driving market revenue.
HAE therapeutics are among the most expensive rare disease treatments globally. Biologic agents such as lanadelumab (Takhzyro) and plasma-derived C1-INH concentrates require chronic administration and involve very high costs per patient. Many healthcare systems, particularly in low- and middle-income countries (LMICs), struggle to provide adequate reimbursement coverage for these expensive orphan-designated medicines. In high-income countries, complex insurance pre-authorization processes, payer restrictions, and budget impact concerns further delay patient access. The high therapy costs create significant economic barriers, particularly for healthcare systems in Latin America, Asia Pacific, the Middle East, and Africa, where HAE is often underdiagnosed and undertreated. These financial barriers restrict market penetration despite a growing unmet need for advanced HAE therapies.
Despite advancements in diagnostic methods, hereditary angioedema remains significantly underdiagnosed globally, particularly in Asia Pacific, Latin America, the Middle East, and Africa. Many patients experience diagnostic delays of several years to over a decade before receiving an accurate HAE diagnosis, exposing them to high risks of fatal laryngeal attacks and unnecessary procedures. A 2024 survey of US physicians indicated that fewer than 38% of patients received an accurate HAE diagnosis within 1–3 years of symptom onset. In the Asia Pacific region, the overall minimal prevalence has been estimated at 0.02 per 100,000, far below expected rates, reflecting significant under-detection due to limited C1-INH testing availability, lack of regional guidelines, and low physician awareness. Until diagnostic gaps are addressed, the full addressable patient population cannot be reached, limiting the market's growth potential.
The approval and commercial launch of sebetralstat (EKTERLY), the first oral on-demand treatment for HAE attacks, represents a transformational opportunity in the HAE therapeutics market. Oral therapies eliminate the need for intravenous or subcutaneous injections, simplifying treatment especially during sudden attacks. Data from the KONFIDENT Phase 3 trial showed median symptom relief within 1.61 hours with oral sebetralstat 300 mg versus 6.72 hours with placebo. Furthermore, once-daily oral berotralstat (ORLADEYO, BioCryst) has already achieved USD 437 million in 2024 sales, with 70% of U.S. HAE patients now choosing oral prophylaxis. This shift toward patient-friendly oral formulations and the convenience of subcutaneous autoinjectors for RNA-targeted agents such as dominators (every 4–8 weeks) is expected to meaningfully expand the treatable patient pool globally, boosting market revenue significantly through 2035.
